A&M Cheese, Co. located at 233 Waggoner Blvd. Toledo, OH 43612; On or about January 9, 2014, the employer did not assure the Combi Casepacker conveyor was deenergized before fixing the tape runner. Employees are required to adjust the tape runner while the equipment is energized, exposing them to amputation hazards.

A&M Cheese, Co. located at 233 Waggoner Blvd. Toledo, OH 43612; On or about January 9, 2014, the employer did not assure employees were protected from accidental start up of the Cuber. Employees are required to change the harps on the Cuber without shutting down the disconnect, exposing them to amputation hazards.
