29 CFR  1910.23(c)(8): The employer did not ensure that the cap (if equipped) and top step of a stepladder were not used as steps.  a) A&M Cheese Co. on or about March 29, 2023, at the address of 233 Waggoner Blvd. Toledo, OH: Sanitation employees tasked with pressure washing equipment were exposed to falls, in that the employer failed to ensure employees did not utilize the top step of 5 ft. self-supporting ladder.

29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) A&M Cheese Co. on or about March 29, 2023, at the address of 233 Waggoner Blvd. Toledo, OH: The employer failed to develop and maintain an effective energy control program to include, employee training and energy isolation procedures. Sanitation employees tasked with cleaning equipment such as but not limited to MS 100 cube/slicer and conveyors without the benefit of energy isolation were exposed to amputation hazards.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) A&M Cheese Co. on or about March 09, 2023, at the address of 233 Waggoner Blvd. Toledo, OH: Production employees tasked with working on south conveyor line were exposed to amputation injuries in that the employer failed to guard ingoing nip points created by the conveyor and its components.  b) A&M Cheese Co. on or about March 09, 2023, at the address of 233 Waggoner Blvd. Toledo, OH: Production employees tasked with working on north conveyor line were exposed to amputation injuries in that the employer failed to guard ingoing nip points created by the conveyor and its components.

29 CFR  1910.212(a)(4):Revolving drum(s), barrel(s), or container(s) were not guarded by enclosure(s) which were interlocked with the drive mechanism so that the barrel(s), drum(s), or container(s) could not revolve unless the enclosure(s) were in place:  a) A&M Cheese Co. on or about March 29, 2023, at the address of 233 Waggoner Blvd. Toledo, OH: Production employees tasked with working near north tumbler without the benefit of the rotating drum being guarded were exposed to amputation hazards. The employer failed to enclose the tumbler with interlocked guarding.
