29 CFR 1926.404(b)(1)(ii): On a construction site, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection.  At the jobsite located at the jobsite located at 4057 N Woodlawn Ave, Wichita, KS: The employer did not ensure that employees were protected by the use of a ground fault circuit interrupter when a 12 volt battery charger was being powered via a yellow AWG 12/2 extension cord. Employees were exposed to a shock hazard.

At the jobsite located at the jobsite located at 4057 N Woodlawn Ave, Wichita, KS: The foot of the left side of the ladder utilized by the employees to access the roof was bent 90 degrees and did not provide a firm support, causing the ladder to be unstable. Employees were exposed to a fall hazard.

29 CFR 1926.501(b)(10): Employee(s) engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8m) or more above lower levels were not protected from falling by a guardrail system, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:  The employer did not ensure that employers performing roofing operations on a low slope roof (4/12 pitch) were utilizing some type of fall protection. Employees were exposed to a fall hazard of approximately ten (10) feet eave to ground.  Gustavo Rodriguez was previously cited for a Serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 846924, citation number 02, item number 01, and was affirmed as a final order on 04/03/2013, with respect to a workplace located at 3719 N Watercress Ct., Maize, Kansas 67101.
