29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:   On or about January 14, 2014, at a worksite located at 475 Victoria Road, Youngstown, OH 44515, during roofing activities including but not limited to removing roof debris and installing a new PVC roof product, the employer did not ensure all employees on the roof were protected from a fall height approximately 15 feet by ensuring an appropriate fall protection system/device was utilized.

29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l):   On or about January 14, 2014, at a worksite located at 475 Victoria Road, Youngstown, OH 44515, during roofing activities including but not limited to removing roof debris and installing a new PVC roof product, the employer did not ensure all powered industrial truck operators were trained and competent prior to operation of the Caterpillar TL642C Rough terrain powered industrial vehicle.
