29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) M&C Construction, on or about June 21, 2023, at the location of 35575 Detroit Road, Avon, OH: The employer did not ensure that employees performing roofing activities on a steep roof were protected from fall hazards. Employees were exposed to fall hazards of 10-12 feet to the ground below while performing roofing activities.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about June 21, 2023 employees were exposed to a fall hazard of approximately 10 feet while performing roofing activities on school located at 35575 Detroit Road, Avon, OH . Employees were using an extension ladder to access the roof and the employer did not ensure they extended at least 3 feet above the upper landing surface.
