29 CFR 1926.451(c)(3): Supported scaffold poles, legs, posts, frames, and uprights shall be plumb and braced to prevent swaying and displacement.    A) Pump jack scaffold, back of house- Employees were exposed to fall hazards of approximately 17 feet while working on a pump jack scaffold that was not braced at the bottom, on or about January 16th, 2014.

29 CFR 1926.1053(b)(1):   Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet  above the upper landing surface to which the ladder was used to gain access:   A) Back of house, ladder to scaffold: Employees were exposed to fall hazards of approximately 17 feet while using an extension ladder to access a pump jack scaffold. The extension ladder that was used did not extend at least 3 feet above the landing, on or about January 16th, 2014.

29 CFR 1926.451(g)(1):   Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.   A) Pump jack scaffold, rear of house - Employees were exposed to fall hazards of approximately 17 feet while working on a pump jack scaffold that was not protected by a complete guardrail system (including end rails) or other means of fall protection in place, on or about January 16th, 2014.   Frame Q LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1), which was contained in OSHA inspection number 897001, citation number 2, item number 1 and was affirmed as a final order on April 28th 2014, with respect to a workplace located at 31 New St. In Englewood Cliffs, NJ.
