On or about January 15, 2014, the employer did not ensure personal protective equipment, such as hard hats were being worn by employees while working underneath other employees on higher levels of the tubular welded frame scaffold.  Employees were exposed to struck by hazards.

On or about January 15, 2014, the employer did not ensure employees performing masonry activities were wearing eye and face protection exposing employees to struck-by hazards and eye injuries.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.    On or about January 15, 2014, the employer did not ensure the electrical extension cord used to energize the concrete mixer used by the employees had a strain relief device.  Employees were exposed to electrical shock hazards.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.   On or about January 15, 2014, the employer did not ensure that a ladder was provided to access or egress the scaffold, exposing employees to fall hazards.

On or about January 15, 2014, the employer did not ensure guardrails were installed on the tubular welded frame scaffold to protect employees from a 15 foot fall hazard.
