Crossbraces were used as a means of access. On or about January 16, 2014, the employer did not ensure a ladder was provided for employees to gain access or to have a means of exiting from the scaffold, exposing employees to fall hazards: a) East side tubular welded frame scaffold, employees working on the 2nd tier. b) South side tubular welded frame scaffold, employees working on the 3rd tier.
