29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:     This violation occurred on June 17, 2014 at a jobsite located at 6223 Velasco Ave. Dallas, TX 75214; on the north side of the construction site,  where a pile of debris had accumulated near the entrance to the jobsite, exposing employees to the hazards of lacerations and/or puncture injuries caused by stepping on nails.

Employees were exposed to fall hazards of approximately 12 feet to the ground below.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access:     This violation occurred on June 17, 2014, at a jobsite located at 6223 Velasco Ave. Dallas, TX 75214; where employees climbed 12-feet on crossbraces from the lower surface level to access the second tier of the south side tubular welded scaffold. The employer did not provide a ladder or other means of direct access to the second tier of the scaffold and employees were exposed to the hazards of falling to the ground below.
