29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a)  Cut Line Department - On or about January 15, 2014, employees operating and/or passing by the outfeed-ends of the following machines were exposed to contact with the unguarded back-side of sliding slearing (shearing) blades where square cuts of sheet-metal were being produced on the five foot slears, i.e., Slear #1, and Slear #2.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:    (a) Cut Line Department, at the Iowa Precision Coil Unwinder, and at SLEAR #1 - On or about January 15, 2014, employees operating Slear #1, and its coil unwinder, were exposed to injury due to their use of a 10 inch long by 1/4 inch OD copper-tube air-nozzle extension, which produced compressed air at greater than (>) 100 psig.

(b) Northwest corner of the Trunk Department - On or about January 16, 2014, employee(s) operating the 10 KVA Western Arctronics Resistance/Spot Welder, Model ET.76, S/N FJ598, were exposed to burns caused by the unintended activation/operation of this welder due to there being no guard over its foot treadle or switch.

(a) ATRP (Air Tight Reduct) Department, Southeast corner - On or about January 16, 2014, employees operating the May Tool Company Model MTC42V3/4 Circle Shear were exposed to electric shock due to the absence of a grounding prong on its male attachment plug.

29 CFR 1910.304(g)(6)(vi)(C)(5): The employer did not ensure that exposed noncurrent-carrying metal parts of cord-and-plug connected equipment that may become energized were grounded when the equipment was used in damp or wet locations by employees standing on the ground:  (b) Boots Department, Sealant Area - On or about January 16, 2014 employees using a Hardcast Mobile Sealant Delivery System to spray Hardcast Duct Sealant onto duct boots were exposed to electric shock because the grounding prong was missing from the male attachment plug on the 25 foot long orange, round, Type SJTW, AWG 16-3 extension cord being used to supply power to the mobile sealant sprayer.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  (a) Boots Department, Sealant Area - On or about January 16, 2014 employees using a Hardcast Mobile Sealant Delivery System to spray Hardcast Duct Sealant onto duct boots were exposed to minor burns because the 25 foot long orange, round, Type SJTW, AWG 16-3 extension cord supplying power to the mobile sealant sprayer was being used as a substitute for the building's fixed wiring.
