29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about January 3, 2024, employees performing maintenance activities on the production floor of Southwark Manufacturing Company located at 2073 North Radenmakers Way, Fremont, Nebraska were exposed to caught-in hazards in that employees were not protected from in-going nip points when degalvanizing the Flager machine rollers. Instances of exposure on January 3, 2024 included but were not limited to: a. Maintenance employee degalvanizing the Flagler 3-in-1 in the Miscellaneous Fittings department by removing the guard while the machine is running and using a screwdriver to scrape the forming rollers clean and b. Maintenance employee degalvanizing the Flagler machine on the duct line by removing the guard while the machine is running and using a screwdriver to scrape the forming rollers clean.
