29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Rough Mill, Rip Line, on or about 1/29/14: Ingoing nip points on the chain conveyor from the pinch roll to the inclined deck were not guarded in two areas at the conveyor chain and sprockets. b) Rough Mill, Rip Line, on or about 1/29/14: Ingoing nip points on the inclined chain conveyor discharge end to the chain conveyor to the rip saw were not guarded at the conveyor chain and sprocket. c) Rough Mill, Rip Line, on or about 2/4/14: Ingoing nip points on the green chain conveyor at the exit end were not guarded at the conveyor chain and sprocket. d) Flooring Line, on or about 2/4/14: Ingoing nip points and rotating parts at the entry end and exit ends of a Leadermac Speedmac 923 moulder were not guarded. e) Flooring Line, on or about 2/4/14: Ingoing nip points, rotating parts and point of operation on a Leadermac Speedmac Moulder 923 were not adequately guarded in that the safety switches provided on the doors were not operational. f) Flooring Line, on or about 2/4/14: Pinch points on the feed rollers for the Q hook table were not adequately guarded. g) Flooring Line, on or about 2/4/14: Ingoing nip points on a belt conveyor for the Saw Optimizer, at the outfeed end, were not guarded. h) Flooring Line, on or about 2/4/14: Ingoing nip points and pinch points on conveying line for the Doucet End Matcher exit end were not guarded where employees are taking boards off one line and moving it along another conveyor and at the conveyor drive belts feeding the boards through the machine. i) Flooring Line, on or about 2/4/14: Ingoing nip points on the Shaw conveyor belt at the outfeed end, were not guarded. Abatement documentation must be submitted for these items.
