OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.  F/V Pacific Producer; on or about 29 January 2014 and times prior thereto; twenty-seven instances were noted where electrical hazards existed:      (a) Upper deck sleeping quarters; an energized overhead florescent lamp had standing water intrusion inside of the protective cover.         (b) Upper deck sleeping quarters, port side, aft lower bunk area, right hand side; an electrical outlet box had exposed energized electrical wires that were not guarded.         (c) Upper deck sleeping quarters, port side, aft upper bunk area, left hand side; an energized electrical outlet switch was missing the protective cover.         (d) Upper deck sleeping quarters, port side, adjacent to toilet; right hand side; exposed energized electrical wires to a hard-wired electrical heater unit was missing the protective cover.          (e) Upper deck sleeping quarters, port side, between two sinks; an energized electrical outlet was missing the protective cover.         (f) Upper deck sleeping quarters, port side, forward upper bunk area, right hand side; an electrical outlet box had energized electrical wires that were not guarded.         (g) Upper deck sleeping quarters, port side, center upper bunk area, right hand side; an electrical outlet box had energized electrical wires were not guarded.         (h) Upper deck sleeping quarters, port side, center upper bunk area, right hand side; an energized electrical outlet was missing the protective cover.         (i) Upper deck sleeping quarters, port side, center upper bunk area, right hand side; an energized incandescent light bulb was not guarded.         (j) Upper deck sleeping quarters, port side, center upper bunk area, right hand side; an energized electrical outlet box was missing the protective cover.         (k) Upper deck sleeping quarters, port side, upper bunk area, left hand side; an electrical outlet box had energized electrical wires that were not guarded.         (l) Upper deck sleeping quarters, port side, upper bunk area, right back side; an energized electrical outlet box was missing the protective cover.         (m) Upper deck sleeping quarters, port side, center lower bunk area, right hand side; an energized incandescent light bulb was not guarded.         (n) Upper deck sleeping quarters, portside, between the bunks; an energized incandescent light bulb was not guarded.         (o) Upper deck sleeping quarters, port side, adjacent to toilet; right hand side; exposed energized electrical wires to a hard-wired electrical heater unit was missing the protective cover.          (p) Upper deck sleeping quarters, starboard side, adjacent to entryway; exposed energized electrical wires to a hard-wired electrical heater unit was missing the protective cover.          (q) Upper deck sleeping quarters, starboard side, adjacent to sink; left hand side; a flexible extension cord was used as permanent wiring.         (r) Entry way to the engine room, right hand side; a 220 volt electrical panel was not labeled indicating its use and purpose.         (s) Bow area, starboard side, refrigerator and compressor area; an electrical box conduit connector was separated from the junction box. Outlet was energized.          (t) Bow area, starboard side, refrigerator and compressor area; an electrical drop light, with an incandescent bulb, was missing the protective cover.          (u) Bow area, starboard side, refrigerator and compressor area; an electrical drop light had damaged to the protective insulation on the cord.          (v) Bow area, starboard side, refrigerator and compressor area; the compressor had an energized electrical panel that was missing its protective cover.         (w) Galley area; seven overhead florescent lights approximately 75 inches from the floor, were missing the protective covers.         (x) Engine room, forward area; an energized electrical outlet box had evidence of arcing and overheating.          (y) Laundry room area; overhead florescent lights approximately 71 inches from the floor, were missing the protective cover.         (z) Laundry room area; an energized electrical outlet box was damaged. A portion of the face plate was broken off.         (aa) Engine room area; an energized electrical bus panel was missing protective cover.      These conditions exposed employees to electrical shock and electrical burn hazards and electrocution.

29 CFR 1910.212(a)(5): Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch.  F/V Pacific Producer, office area, on or about 29 January 2014 and times prior thereto; a Markel, Model HF3185, electric heater fan blade was not guarded. This condition exposed employees to a caught in hazard.

29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6.  F/V Pacific Producer, bow, starboard side, refrigerator and compressor area; on or about 29 January 2014 and times prior thereto; compressed gas cylinders of chlorodifluoromethane were not stored with the protective cap installed. This condition exposed employees to inhalation of toxic substances.
