Power source was used to power multiple heaters, exposing employees to electrical and fire hazards.   b) Wheelhouse - On or about October 25, 2022, and times prior thereto, employees utilized a relocatable power tap connected in series to a second relocatable power tap on the wall facing the bow of the vessel.  Power source was used to power a printer, exposing employees to electrical and fire hazards.    c) Captains Room - On or about October 25, 2022, and times prior thereto, an employee utilized a relocatable power tap for power of appliances to include microwave and fridge.  The relocatable power tap had been permanently affixed to flexible cord attached to the wall powering the outlet for use, exposing employees to electrical and fire hazards.   d) Room 2 - On or about October 25, 2022, and times prior thereto, the Owner and Managing member of the pacific producer had permanently affixed a relocatable connected in series to a second relocatable power tap on the wall with sink and mirror.  The relocatable power taps were for use of powering devices such as lighting fixtures and phone chargers, exposing employees to fire and electrical hazards.  e) Oficina - On or about October 25, 2022, and times prior thereto, the Owner and managing member of the Pacific Producer had permanently affixed and utilized multiple relocatable power taps as an additional outlet source for microwave, chargers, and computer equipment.   g) Main Deck/Outside Galley - On or about October 25, 2022, and times prior thereto, an outlet receptacle was not used in accordance with listing and labeling when installed and used with series connection of extension cords and was placed on the floor when utilized as a power source for refrigerators, exposing employees to electrical and fire hazards.    h) Main Deck/Outside Galley - On or about October 25, 2022, and times prior thereto, extension cords were connected together in series outside, and a second extension cord was modified at the female end and hardwired for permanent installation into an outlet receptable, exposing employees to electrical and fire hazards.

29 CFR 1910.303(c)(3)(i):  All splices and joints and the free ends of the conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose.  a) Wheelhouse - On or about July 20, 2022, and times prior thereto, conductors of TSGU-4 10AWG/4 power cable were spliced to conductors of a BC-5W2 marine boat cable with use of a splice connector and were not covered with an insulation, exposing employees to electrical and fire hazards.   b) Wheelhouse - On or about July 20, 2022, and times prior thereto, conductors of a BC-5W2 14AWG/2 marine boat cable were spliced with conductors of a blue strobe light and a bell alarm manufactured by Aqualarm for hardwiring and were not covered with insulation, exposing employees to electrical and fire hazards.   c) Stateroom 4 - On or about July 20, 2022, and times prior thereto, electrical tape was used to cover electrical conductors for flexible wiring of an exhaust fan and heater spliced to a BC-522 14AWG/2 flexible cord exiting a hole in the ceiling providing power from a circuit breaker, exposing employees to electrical and fire hazards.  d) Galley Kitchen - On or about September 29, 2022, and times prior thereto, a braided flexible cord on top of the oven exhaust was observed spliced at ends with wire gauge caps installed and electrical tape placed over parts of the conductors, exposing employees to electrical and fire hazards.    e) Pump room - On or about September 29, 2022. and times prior thereto, flexible cords were observed spliced for a quadplex receptacle unit adjacent to the exhaust stack.  Electrical tape was placed over conductors, exposing employees to electrical and fire hazards.  f) Stateroom 2 - On or about October 25. 2022, and times prior thereto, the free ends of conductors of an overhead light receptable located above the sink were covered with electrical tape, exposing employees to electrical and fire hazards.    g) Stateroom 2 - On or about October 25, 2022, and times prior thereto, a 10AWG braided sheathed cable had free ends of three conductors exposed and covered with electrical tape, exposing employees to electrical and fire hazards.   h) Stateroom 2 - On or about October 25, 2022, and times prior thereto, electrical conductors of a 14AWG flexible cord were spliced to electrical conductors in an open junction box receded into the wall near the ceiling and were not covered with insulation, exposing employees to electrical and fire hazards.  i) Stateroom 2 - On or about October 25, 2022, and times prior thereto, a light fixture at the ceiling was hardwired with the conductors of the fixture and the conductors of a light switch below spliced and connected to 14 AWG flexible cord secured to the ceiling area and was not covered with insulation, exposing employees to electrical and fire hazards.   j) Stateroom 2 - On or about October 25, 2022, and times prior thereto, a 14 AWG flexible cord painted white was observed attached to the ceiling and had conductors spliced without proper insulation covering the open ends of the conductors, exposing employees to overhead electrical hazards.   k) Stateroom 8 Storage - On or about October 25, 2022, and times prior thereto, a 14 AWG flexible cord was spliced and connected to a light fixture and light switch receptable at the ceiling area and the conductor ends were covered with electrical tape, exposing employees to electrical and fire hazards.   l) Stateroom 9 - On or about October 25, 2022, and times prior thereto, a 14 AWG flexible cord was observed spliced with the conductors covered with painters' tape, exposing employees to electrical and fire hazards.

29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and cables were run through holes in walls, ceilings, or floors:  a) Wheelhouse - On or about July 20, 2022, and times prior thereto, multiconductor flexible cords of various gauges were used as a substitute for fixed wiring and was observed attached with zip ties along the walls and ceiling.  The cables exited the wheelhouse to the exterior through two holes in the wall above the door frame, exposing employees to electrical and fire hazards.  b) Stateroom 4/Bunk Wall - On or about July 20, 2022, and times prior thereto, 14 AWG flexible cable painted white was observed running from the head wall to side wall inside of the crew bunk space to provide power to an outlet receptacle installed providing power supply to a lamp fixture, exposing employees to electrical and fire hazards.    c) Captains Room - On or about October 24, 2022, and times prior thereto, 10AWG braided cable and 14AWG braided cable, both painted white, were attached together with the use of metal cable clamps and zip ties.  Both cables were used as substitute for permanent wiring and were running into the room from the exterior and was observed attached to the ceiling and walls for connection to light switch, lighting fixture, outlet receptables, a heater shell, and junction boxes, exposing employees to electrical and fire hazards.    d) Stateroom 2 - On or about, October 25, 2022, and times prior thereto, 10AWG braided cable and 14 AWG flexible cable had been painted white and were observed running along the walls and ceilings secured with use of metal cable clamps and screws.  The cables were run through holes in the wall and were used as a substitute for fixed wiring providing power supply to a light switch, light fixture, and outlet receptables, exposing employees to electrical and fire hazards.    e) Stateroom 8 - On or about October 25, 2022, and times prior thereto multiple 14 AWG flexible cords hardwired to light receptable were observed exiting the room through holes in the wall, exposing employees to electrical and fire hazards.   f) Stateroom 8 - On or about October 25, 2022, and times prior thereto, a black 14AWG flexible cord observed for power supply of an outlet fixture installed for use was observed running through a hole in the wall, exposing employees to electrical and fire hazards.    g) Stateroom 8 - On or about October 25, 2022, and times prior thereto, a flexible cord used for wiring of a dual outlet receptable, and light fixture unit was observed running through a wall opening, exposing employees to electrical hazards.   h) Stateroom 8 - On or about October 25, 2022, and times prior thereto, a flexible cord attached to a light fixture was observed running through a hole in the ceiling, exposing employees to electrical and fire hazards.    i) Stateroom 8 - On or about October 25, 2022, and times prior thereto, flexible cable painted white ran along the exterior wall and into a hole to the interior for connection of a sub-fed control panel, exposing employees to electrical and fire hazards.    j) Stateroom 9 - On or about July 20, 2022, and times prior thereto, a flexible 14 AWG painted cord was observed entering through a hole above the sink wall and exiting in a hole in the ceiling, exposing employees to electrical and fire hazards.    k) Stateroom 10 - On or about July 20, 2022, and times prior thereto, flexible cords painted white had been ran through holes placed in the walls, exposing employees to electrical and fire hazards.

29 CFR  1910.305(g)(1)(iv)(E): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used:  Where concealed behind building walls, ceilings, or floors.  a) Stateroom 7 - On or about July 20, 2022, and times prior thereto, a 14 AWG flexible cord painted white was observed running along the walls and was concealed behind the wood bunk assembly installed on the wall, exposing employees to electrical and fire hazards.   b) Stateroom 9 - On or about July 20, 2022, and times prior thereto, a flexible cord used for power supply to outlet receptacles was observed running into a hole opening behind wall of storage compartment next to bunk space and was concealed until observed exit at the back of the storage unit at the head of the top bunk, exposing employees to electrical and fire hazards.   c) Main Deck/Galley - On or about October 25, 2022, and times prior thereto, and times prior thereto, the employer had installed and used flexible cables and cords sourced from control panels in the wheelhouse underneath the main decking to provide power to lower levels, exposing employees to electrical and fire hazards.
