29 CFR 1910.141(a)(3)(i): Places of employment were not kept clean to the extent that the nature of the work allowed  F/V Pacific Producer, Upper deck crew sleeping quarters:  On or about 29 January 2014 and times prior thereto, the toilet facilities, starboard and port side of vessel, were unsanitary and not functional.  This condition exposed employees to biological hazards, sickness and communicable diseases.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met.   F/V Pacific Producer:  On or about 29 January 2014 and times prior thereto, the employer did not provide a Hazard communication program for employees working on the vessel, to include, but not limited to, Ammonia, Freon, and other pressurized gases, grease, fuels and oils. The program must include training on the Global Harmonization System requirements.  These conditions exposed employees to contact with toxic, caustic or noxious substances.

29 CFR 1915.89(b):  The employer did not establish and implement a written program and procedures for lockout and tags-plus systems to control hazardous energy during the servicing of any machinery, equipment, or system in shipyard employment.  F/V Pacific Producer, engine room:  On or about 29 January 2014 and times prior thereto, employees working on the main engine without energy isolation procedures.  This includes maintenance on the engine electrical control panel. This condition exposed employees to caught in, electrocution and burn hazards.
