29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition. a. At the site, the employer did not ensure a Reallift forklift serial number 120419591 was taken out of service when the lights were not working properly. Rural King was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178 (p)(1), which was contained in OSHA inspection number 98177, citation number 1, item number 1 and was affirmed as a final order on November 23, 2010, with respect to a workplace located at1920 S Mt Zion Rd, Decatur, IL 62521.

Case number 59-1, LB on 9/4/13, did not include the cause of the injury. 2. Case number 59-2, CR on 11/10/13, did not include the cause of the injury.
