Powered industrial lift trucks were observed entering and exiting dropped trailers and no wheel chocks were in place.

29 CFR 1910.178(m)(7): Brakes were not set and/or wheel blocks were not in place to prevent movement of trucks, trailers, or railroad cars while loading and unloading: On or about January 30, 2014, the employer did not ensure trailers had wheel chocks in place before powered industrial trucks entered the trailers. Powered industrial lift trucks were observed entering and exiting dropped trailers and no wheel chocks were in place. The company has not evaluated the work practice to determine if fixed jacks may be necessary to support the semitrailer during loading and unloading when the trailer is not coupled to a tractor.
