29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) At the Bergandi chain weaver #1, on or about July 31, 2014, and at times prior to, the wire benders were not adequately guarded in that the side of the rollers where the wire was fed into the machine did not have guards, exposing employees to a pinch point hazard.       b) At the Bergandi chain weaver #2, on or about July 31, 2014, and at times prior to, the wire benders were not adequately guarded in that the side of the rollers where the wire was fed into the machine did not have guards, exposing employees to a pinch point hazard.      c) At the Bergandi chain weaver #3, on or about July 31, 2014, and at times prior to, the wire benders were not adequately guarded in that the side of the rollers where the wire was fed into the machine did not have guards, exposing employees to a pinch point hazard.

29 CFR 1910.303(b)(7)(iv): Electrical equipment had damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating.      a) At the warehouse, on or about July 31, 2014, and at times prior to, an electrical box located on the top of an electric motor for a platform stretch wrapper was not closed adequately and the electrical wires were exposed, exposing employees to minor electrical shock hazards.      b) At the warehouse, on or about July 31, 2014, and at times prior to, the cover for an electric motor for a platform stretch wrapper was damaged and the electrical wires were exposed, exposing employees to minor electrical shock hazards.

29 CFR 1910.305(g)(2)(ii): Flexible cords were not used only in continuous lengths without splice or tap:  a) At the warehouse, on or about July 31, 2014, and at times prior to, the extension cord for the Infrapak Platform Stretch Wrapper located by the rack system had the electrical wires spliced together using black electrical tape, exposing employees to minor electrical shock hazards.
