The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: In the boiler room, main Hurst boiler that provides steam for the facility, on or about February 5, 2014 and at times prior thereto, an employee engaged in maintenance on the Hurst boiler (Model: Cyclone, Serial: V86-150-TF21) steam supply lines, was exposed to hazards associated with unexpected release of stored steam.

(b) Steam pipes routed along the Northeast wall, main production area: while walking alongside the wall next to the steam pipes that had inadequate insulation. (c) Boiler room, West wall: while conducting routine maintenance on the reservoir tank (make model not found) that does not have any insulation on the supply lines of the tank.

(b) Compressor room, Northeast corner wall: while turning on/off the Ingersoll-Rand compressor (Model:2545E10V, Serial:902846) that has a missing cover on the electrical supply junction box; exposing electrical conductors. (c) In the main production area, Unipress machine: while using the Unipress machine (Model: STF, Serial: 0575509) that has a bent metal cover on the supply junction box; exposing electrical conductors.
