The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    In the boiler room, main Hurst boiler that provides steam for the facility, on or about February 5, 2014 and at times prior thereto, an employee engaged in maintenance on the Hurst boiler (Model: Cyclone, Serial: V86-150-TF21) steam supply lines, was exposed to hazards associated with unexpected release of stored steam.

29 CFR 1910.264(c)(4)(iii)(a): All steam pipes that are within 7 feet of the floor or working platform, and with which the worker may come into contact, were not insulated or covered with a heat-resistive material or otherwise properly guarded:     On or about on or about February 5, 2014, and at times prior thereto, employees were exposed to burn hazards from steam pipes that were not insulated or properly guarded:     (a) Garment touch-up station, touch-up press machine (make and model not found): while using the press machine; which had inadequate insulation on the steam pipes.     (b) Steam pipes routed along the Northeast wall, main production area: while walking alongside the wall next to the steam pipes that had inadequate insulation.     (c) Boiler room, West wall: while conducting routine maintenance on the reservoir tank (make model not found) that does not have any insulation on the supply lines of the tank.

In completed installations, each outlet box did not have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear:     On or about on or about February 5, 2014, and at times prior thereto, employees were exposed to open junction boxes with live electrical conductors:     (a) In the main production area, on the Southeast wall: while turning on a light switch that had a broken cover; exposing electrical conductors.     (b) Compressor room, Northeast corner wall: while turning on/off the Ingersoll-Rand compressor (Model:2545E10V, Serial:902846) that has a missing cover on the electrical supply junction box; exposing electrical conductors.     (c) In the main production area, Unipress machine: while using the Unipress machine (Model: STF, Serial: 0575509) that has a bent metal cover on the supply junction box; exposing electrical conductors.
