29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew: In the Gold Department on the Multi-Vac R80 (tongue line): The employer failed to ensure that all employees were afforded a level of protection of personal Lockout / Tagout while cleaning was performed prior to shut down. Affected employees assist in cleaning the machine, prior to cleaning the machine, unused bags are removed and cycled through then the machine is locked out and the employees clean the machine.

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: On the multvac R80 in the Gold Department- the employer failed to ensure that guarding was adequate to prevent employees from placing any part of their body in the point of operation. On or about 1-21-2014 an employee was injured while assisting to clean the machine. The employee placed their right hand in to the machine to place a plastic bag over the forming dies of the multivac; as the machine cycled the dies came together crushing the employee's hand.
