29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices in the following instances:    A) Machinery Room - On or about November 5, 2019, the employer failed to document that equipment complied with employer's chosen RAGAGEP such as but not limited to IIAR 2 - Standard for Safe Design of Closed Circuit Ammonia Refrigeration Systems (2008) Section 13.1.6 incorporates ANSI/ISEA Z358.1- American National Standard for Emergency Eyewash and Shower Equipment (2014) Sections 5.4.1 and 5.4.2 by reference, when the employees did not have access to an eyewash and body shower unit located outside (within 55 feet or reachable within 10 seconds) of the engine/machinery room, exposing employees to toxic and fire hazards in the event of an ammonia release.    B) Machinery Room - On or about November 7, 2019, the employer failed to document that electrical conduit into the electrical room and piping (e.g. wash lines) into the boiler room penetrated the interior walls of the engine/machinery room complied with employer's chosen RAGAGEP such as but not limited to ASHRAE 15 (2015) Section 8.12(f), and IIAR 2 (2008) Section 13.1.5.2, when the wall penetrations were not tightly sealed, exposing employees to toxic and fire hazards in the event of a release.
