29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program, as described in paragraphs (c) through (o) whenever employee noise exposures equal or exceeded an 8-hour time-weighted average sound level (TWA) of 85 decibels measured on the A scale (slow response) or, equivalently, a dose of fifty percent. This violation was discovered on or about February 5, 2014, where the employer does not administer a hearing conservation program for employees working in the shipping and crating areas who are exposed to noise levels that exceed an 8-hour TWA of 85dBA.

This violation was discovered on or about February 5, 2014, where the employer does not develop, implement, and maintain a written hazard communication program for employees working in the warehouse and painting department when working with chemicals such as, Methyl Ethyl Ketone, and paint containing: Methyl Isobutyl Ketone, Hexamethylene Diisocyanate, Hexamethylene Diisocyanate Polymer, 1-Butanol, Ethylbenzene, n-Butyl Acetate, and t-Butyl Acetate.
