29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):     The employer failed to ensure that adequate Job Safety Analysis (JSA) was conducted prior to allowing employees to work on the job site.  The JSAs the company has completed do not address all of the hazards involved in the operations.  In two separate accidents, employees were wearing PPE not adequate for the operation being conducted.      1.  An employee tack welding was wearing protective glasses and the cinder from his weld went up in between the glasses and his face burning him.     2.  An employee working in the galvanizing area was exposed to and inhaled ammonium chloride when the hose to the machine which was adjacent to his area came off of the machine spewing the product onto the worker which was not wearing any form of PPE.
