29 CFR 1910.157(f)(16): Evidence was not maintained and provided upon request which showed that the required hydrostatic testing of portable fire extinguishers had been performed at the time intervals shown in Table L1:  (a) US Border Patrol Tucson Station, 2430 S Swan Rd., Tucson, AZ 85711:  On or about 12 February 2014, and at times prior thereto, the employer did not maintain a record of required hydrostatic tersting of the portable fire extinguishers mounted and available for use throughout the facility, exposing the employees to hazards.

29 CFR 1910.159(c)(2): The employer did not properly maintain the installed automatic sprinkler system:  (a) US Border Patrol Tucson Station, 2430 S Swan Rd., Tucson, AZ 85711:  On or about 12 February 2014, and at times prior thereto, the employer did not maintain the automatic sprinkler system by conducting a main drain flow test annually or operating the inspector's test valve every other year to ensure that the system operates properly, exposing the employees to hazards.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:  (a) US Border Patrol Tucson Station, 2430 S Swan Rd., Tucson, AZ 85711:  On or about 12 February 2014, and at times prior thereto, electrical circuits were not identified as to location or purpose, in these instances, exposing employees to hazards:  (1) First Instance: at multiple locations throughout the facility red-colored mushroom type emergency stop push buttons were installed on the walls and these were not marked as to indicate its purpose or disconnecting circuits;  (2) Second Instance: electrical panel ER2-152A had circuit breaker switches installed as 20 amperes each and ganged together for #19, 21, and 23 but these were not listed on the panel legend;  (3) Third Instance: electrical panel ER1-152A had breaker slots 15 through 23 blanked but the panel legend indicated switches #17, 19, and 21 were listed as temporary power to the IOCC building.

29 CFR 1904.7(b)(3): The employer did not correctly classify the following work-related injuries or illnesses on the OSHA Form 300 or equivalent.   (a) US Border Patrol Tucson Station, 2430 S Swan Rd., Tucson, AZ 85711: On or about 12 February 2014, and at times prior thereto, the employer did not correctly record the following instances of work-related injuries or illness on the OSHA 300 Log:  (1) First Instance: Case # 132302812, wherein an agent, on December 1, 2013, transported an arrestee who had a positive culture for Group A streptococcus which resulted in the agent experiencing three days away from work, and the OSHA 300 Log for 2013 incorrectly had an entry in column M1 (injury) instead of column M6 (other illness);  (2) Second Instance: Case # 132305189, wherein an agent, on January 19, 2014, impaled his left knee on a cholla cactus spine and experienced days away from work, and the OSHA 300 Log for 2014 incorrectly did not record any number of days away from work in column K.    Abatement Certification and Documentation is required
