29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about February 18, 2014 and times prior thereto, employees of Carlos Ibarra Construction were observed stapling Tyvek Wrap to the frame of the residence being constructed at or near 1924 Hilldale Road in Alexander, Arkansas. An employee was observed working from ground level without the use of a protective helmet with an employee working overhead stapling.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: On or about February 18, 2014 and times prior thereto, employees of Carlos Ibarra Construction were observed installing ceiling joists and stapling Tyvek Wrap to the frame of the residence being constructed at or near 1924 Hilldale Road in Alexander, Arkansas. The employer had not provided a training program for each employee who was observed exposed to fall hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about February 18, 2014 and times prior thereto, employees of Carlos Ibarra Construction were observed installing ceiling joists and stapling Tyvek Wrap to the exterior of the frame for the residence being constructed at or near 1924 Hilldale Road in Alexander, Arkansas. The employer did not ensure that each employee working and walking on the wood framing without the use of fall protection, exposing employees to a fall distance of 9 feet to the ground level below. Carlos Ibarra Construction was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13) , which was contained in OSHA inspection number 835763, citation number 1, item number 1 and was affirmed as a final order on March 28, 2013, with respect to a workplace located at 104 Vienne Place in Maumelle, Arkansas.
