29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: On or about March 22, 2018, at the worksite located a 9 Tupelo Court in Little Rock, Arkansas, a safe method such as but not limited to a ladder was not provided for accessing the top plate of the residential framing. Employees used 2 x 4's attached to the wall framing that had excessive climbing space between stepping points and a narrow width. Employees were exposed to slips and falls from elevations.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about March 22, 2018, at the worksite located at 9 Tupelo Court in Little Rock, Arkansas, employees standing and traveling on the top plate of the residential framing while engaged in wall plumbing tasks, were not donned in fall protection and were not protected from falling to lower ground. Employee were exposed to unprotected falls of greater than 6 feet. Carlos Ibarra Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 958928, citation number 2, item number 1 and was affirmed as a final order on March 26, 2014, with respect to a workplace located at or near 1924 Hilldale Road in Alexander, Arkansas.
