29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Boulder Roofing Inc. at 1075 11th Street, Boulder, CO 80302: On and before February 18, 2014, the exposing employer did not ensure that employees were using fall protection when shingling a two story residential roof with a 6 in 12 pitch. The condition exposed employees to fall hazards of up to 18 feet. Boulder Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent, 29 CFR 501(b)(13), which was contained in OSHA inspection number 936278, citation number 1, item 2, issued September 4, 2013 and became a final order on October 30, 2013, with respect to a workplace located at 2195 Stony Hill, Boulder, CO 80305.
