29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:    100 block of Steiner Rd., Lafayette, LA:  On or about February 18, 2014, the employer failed to ensure that the working level of the scaffold was fully planked.  Employees were working from the second level of a fabricated frame scaffold on a working platform consisting of one 2" x 10" plank.  The gap between the planking and guardrail supports were approximately 28".

29 CFR 1926.451(b)(5)(i): The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end.    100 block of Steiner Rd., Lafayette, LA:  On or about February 18, 2014, the employer allowed employee(s) to work from a scaffold platform that extended over the supports by 32 inches.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation.    100 Block of Steiner Rd., Lafayette, LA:  On or about February 18, 2014, the employer did not ensure that the fabricated frame scaffolding was resting on baseplates and mudsills.  One scaffold pole was resting on uneven rocky terrain, another pole was elevated using bricks and a piece of wood, and some of the poles were on bare ground.

Employee(s) were climbing the crossbraces  and end frames to access the upper level.
