29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a. On or about February 20th, 2014, at the above addressed job site employees were exposed to an approximate 19 foot fall when using an extension ladder which did not extend at least 3 feet above the upper landing surface while gaining access to the roof.
