29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: Alejandro Hernandez - Osario dba HO Company had employees exposed to 10.5 ft. fall hazards, while accessing and exiting the roof via an extension ladder that only extended 1.5 ft. above the roof surface, on a residential construction site in Savoy, Illinois. Alejandro Hernandez - Osario dba HO Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 417243, citation number 1, item number 4A and was affirmed as a final order on July 12, 2012, with respect to a workplace located at corner of Sunflower Court and Tickseed Avenue, Savoy, IL 61874.
