29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) On and at times prior to February 27, 2014, employees were pulling and pushing wooden pallets through a band saw Model No. B54D and were not protected from the rotating blade. Employees were not protected by distance or guarding from the point of operation on the pallet dismantler. b) On and at times prior to February 27, 2014, employees were reaching and pushing wooden pallets through a band saw Model No. B54E and were not protected from the rotating blade. Employees were not protected by distance or guarding from the point of operation on the pallet dismantler. Manufacturer's Pallet and Disposal, Inc. was previously cited for a violation of this Occupational Safety and Health Standard or an equivalent Standard which was contained in OSHA Inspection Number 457954, Citation No. 1, Item No. 1, issued on 08/10/2012, and affirmed as a Final Order on 09/05/2012, with respect to a workplace located at 605 Industrial Drive, Waupaca, Wisconsin 54981.
