29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used. On or about February 25, 2014, on the west side of the work site, employees engaged in framing activities utilized an extension cord with the outer sheathing split exposing internal wiring exposing employees to electrical hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about February 25, 2014, on the west side of the construction site, employees engaged in framing activities without fall protection were exposed to a fall hazard of approximately 11 feet to the lower level.
