29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph.     a) 2nd floor center:     Employees used an energized electric panel without cover to operate power supply on the floor. Live parts of the electric panel and circuit breakers were not guarded against accidental contact by employees. On or about 02/25/14. 29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge.  a) Stairway between 1st floor and 2nd floor:     Employees used a stairway to travel between 1st and 2nd floor. The stairway rise to about 10 feet height and was not equipped with a stairrail system along its unprotected side. On or about 02/25/14. 29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed.   a) East side of jobsite, side entrance to 1st floor of house:      Employees folded and leaned a 5-foot A-frame stepladder on an elevated entrance to access the 1st floor. On or about 02/25/14.   Note:  Because abatement of this violation is already documented in the inspection case file, the employer need not submit certification of abatement for this violation as normally required by 29 CFR 1903.19.

The tools were connected to an extension cord, which was routed to a nearby electric outlet in the same room. The extension cord was missing a ground pin. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, ITS EQUIVALENT, OR ESSENTIALLY SIMILAR STANDARD, 29 CFR 1926.404(f)(6), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 910524, CITATION NUMBER 1, ITEM NUMBER 2, ISSUED ON 09/20/2013 WITH A FINAL ORDER DATE OF 10/14/2013.
