Employees were exposed to fall hazards in excess of 10 feet to the lower level because they were not protected from falling by means of Personal Fall Arrest System, Guardrails, Safety Nets or Controlled access zone.  The floor at the entry/exit was removed exposing the floor joists and basement below. Employees were utilizing this area as a means of entry/exit into the project.

Brooklyn, NY 11210.
