OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:      a) Sterling Technologies / 10047 Keystone Drive, Lake City, PA --- Resin Storage Rack Area: On or about March 3, 2014, employees were exposed to struck-by and crush hazards from storage racks that were not maintained in a safe, reliable, and stable condition in that the storage racks were not level, missing components, and were not anchored to the floor.  In the resin storage rack area, the deficiencies included but were not limited to: (1) the racks base plates were not anchored to the floor. (2) the rack was modified/rearranged in a manner not within original design configuration in that the rack was missing at least 2 horizontal supports (3) rack was not level/plumb and was out of alignment.    b) Sterling Technologies / 10047 Keystone Drive, Lake City, PA--- Resin Storage Rack Area: On or about March 3, 2014, employees were exposed to struck-by and crush hazards from improper storage an alignment of items being stored on storage racks. In the resin storage rack area, pallets were not placed/stacked in a stable manner on the racks in that only two corners of a pallet were resting on the horizontal supports.            c.) Sterling Technologies / 10047 Keystone Drive, Lake City, PA  --- Resin Storage Rack Area and Storage Rack Adjacent to Column B7 --- On or about March 3, 2014, employees were exposed to struck-by and crush hazards from potentially overloaded storage racks in that the load rating for the storage racks were not prominently posted on the storage rack system.  Storage racks that did not have a posted load rating include the Rack system for the Resin Storage Area and the Storage Rack Adjacent to Column B7.       d.) Sterling Technologies / 10047 Keystone Drive, Lake City, PA--- Storage Rack Adjacent To Column B7:  : On or about March 3, 2014, employees were exposed to struck-by and crush hazards from storage racks with damaged structural components. The storage rack adjacent to column B7 had damaged structural components and was being utilized to store items that included but were not limited to molds.  The damaged components included but were not limited to 2 vertical posts nearest to the aisle were bent (2) The horizontal support connecting the two vertical columns was bent inward towards the interior of the storage rack  (3)A section of the horizontal support was shearing off on of the vertical supports.

29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times:  a) Sterling Technologies / 10047 Keystone Drive, Lake City, PA: On or about March 3, 2014 the emergency exit light on the South East section of the production area was not illuminated.  b) Sterling Technologies / 10047 Keystone Drive, Lake City, PA: On or about March 3, 2014 the alarm speakers on the South wall near the entrance into the production floor were covered in clear tape which would affect the sound of the alarm if activated.

This prevents them from being readily accessible by employees.
