29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used: Employees work with and around sources of heat including, but not limited to, rotational molding machine ovens. Employees have not received training on heat related hazards.

29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tank cars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1, which is incorporated by reference as specified in CFR 1910.6, Section 5.8.4, all cylinders were not secured to prevent falling or rolling: a.) Rotational Oven Area - On or about July 27, 2015 a compressed gas cylinder of propane, located on platform 1 for molding machine 280, was free standing and not properly secured to prevent falling; and b.) Rotational Oven Area - On or about September 4, 2015 a compressed gas cylinder of propane, located on the floor next to molding machine 220, was free standing and was not secured by any device to prevent the cylinder from falling, rolling, or getting knocked over.
