29 CFR 1910.212(a)(5): Fan blade guard(s) were not provided where the periphery of the blades was less than seven feet from the floor or working level:     a) Sterling Technologies, Inc. / 10047 Keystone Dr., Lake City, PA: On or about April 5, 2015 and times prior thereto, a fan on the 280 rotational molding machine oven was not guarded inside of the machine. The portion of the fan inside the 280 rotational molding machine oven was less than 7 feet from the floor.  Employees entered the machine in order to scrape and clean the floor approximately once per shift.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees  A.) Sterling Technologies, Inc. / 10047 Keystone Dr., Lake City, PA: On or about April 5, 2015 and times prior thereto, Employees periodically conducted servicing and maintenance, such as cleaning the floors, inside of the Rotational Molding Machines.  The Rotational Molding Machines were not locked out prior to employees conducting the servicing and maintenance inside the machines.  B.) Sterling Technologies, Inc. / 10047 Keystone Dr., Lake City, PA: On or about April 5, 2015 and times prior thereto, an employee entered the 280 Rotational Molding Machine to retrieve a piece of equipment.  The 280 Rotational Molding Machine was not deenergized and locked out prior to the employee entering the machine.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  a) Sterling Technologies, Inc. / 10047 Keystone Dr., Lake City, PA: On or about April 5, 2015 and times prior thereto, employees periodically performed servicing and maintenance on various machines including, but not limited to, Rotational Molding Machines.  The lockout/tagout procedures did not include specific information regarding the scope, purpose, authorization, rules and techniques to be utilized for the control of hazardous energy; a specific statement of the intended use of the procedure; specific procedural step for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; the magnitudes of the energy sources; specific procedural steps for testing machines and equipment to verify the deenergization; specific requirements for notification of affected employees;  specific procedures for reenergization of the machines; specific requirements for inspections of work area prior to reenergizing equipment; and specific requirements for notification of employees prior to reenergizing equipment.
