29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    a) Extruder/Production Area: Winder machine operators were exposed to continuous noise levels at 74.17%, 96.1%, and 137.8% of the 8-hour time weighted average sound level of 85 decibels.  The equivalent dBA levels of 74.17%, 96.1%, and 137.8% are approximately 87.8 dBA, 89.7 dBA, and 92.4 dBA respectively.  The samples were performed for 470, 480, and 476 minutes during the day shift on March 7th, 2014.  Zero exposure was assumed for the unsampled period of time.  The employer did not administer a continuing, effective hearing conservation program, on or about March 7th, 2014.
