29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. a) In the Water Pump and Disinfection Bldg 1207, PRTC project site, Andersen AFB, at the install back-up generator the electrical cord powering the concrete mixer had torn insulation.

29 CFR 1926.502(b)(2)(i): Midrails were not installed at a height midway between the top edge of the guardrail system and the walking/working level. a) At the Water Pump and Disinfection Bldg 1207, PRTC project site, Andersen AFB, employees were exposed to an approximate eight foot fall through a 30 inch gap in the guard rail system.
