29 CFR 1926.95(c): Personal protective equipment was not of safe design and construction for the work to be performed: Police Station Project: Employees were observed using cotton work gloves while using wet concrete containing Portland cement. Wet concrete residue was observed on a worker's forearms and hands. Employees are exposed to skin irritation or and chemical burn hazards.

29 CFR 1926.451(b)(1)(i):Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) shall be installed so that the space between adjacent units and the space between the platform and the uprights is no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary (for example, to fit around uprights when side brackets are used to extend the width of the platform). Front of the project: Scaffold planking used to access the project had measured gaps in excess of 1" between various sections of planking in the front of the project. The largest gap was 4.5". Employees were exposed to tripping hazards.
