29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer does not ensure that machine guarding is provided to protect the operator and other employees in the machine area from hazards such as those created by moving parts. This violation occurred on or about March 12, 2014, in Building A where a saw operator was exposed to struck-by hazards when operating a H90-A Hem saw (SN: 855004) without a guard over the unused portion of the blade. Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that machine guarding is provided to protect the operator and other employees in the machine area from hazards such as those created by moving parts.

This violation occurred on or about March 12, 2014, in Building A where a shear operator was exposed to struck-by hazards when operating an Accur Shear (SN: 3529) without adequate guarding. Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that machinery is guarded to prevent employees from having any part of their body in the danger zone during operating cycles.

This violation occurred on or about March 12, 2014, at the facility where employees were using chemicals such as, but not limited to, VP-Tech coolant when cutting metal without providing training on the workplace labeling system and the safety data sheet hazard information. Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the hazard communication program include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer and the safety data sheet hazard information.
