29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:  (a) In the conference room the Whirlpool ED19HK refrigerator was not grounded.  The grounding pin of the refrigerator cord had been defeated by plugging the cord into a two prog un-grounded extension cord leaving the pin exposed.
