Ventra Sandusky, LLC located at 3020 Tiffin Ave. Sandusky, OH 44870; On or about March 14, 2014, the employer did not ensure employees were protected while working on hot molds in the multicolor press. Employees are required to perform mold cleaning activities and are exposed to burns from molds of up to 180 degrees Fahrenheit.

Ventra Sandusky, LLC located at 3020 Tiffin Ave. Sandusky, OH 44870; On or about March 14, 2014, the employer did not utilize written energy control procedures on the Engel #MU36605 Multicolor Press. Employees are required to perform mold cleaning activities; including fixing damaged steel and did not lock out the mold heater, exposing them to burn injuries from hot plastic.

Ventra Sandusky, LLC located at 3020 Tiffin Ave. Sandusky, OH 44870; On or about March 14, 2014, the employer did not assure employees were protected from accidental start-up of the Engel #MU 36605 Multicolor Press. Employees are required to perform mold cleaning activities and do not lock out the pneumatic energy of the valve gates, exposing employees to possible burn hazards from hot plastic.
