29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    a) Ventra Sandusky, LLC, 3020 Tiffin Ave. Sandusky. On or about July 13, 2015. The employer did not assure periodic inspections were conducted for employees working in section M2 of the facility who were performing lockout tagout on a plastic injection molding machine.

On or about July 13, 2015. The employer did not assure that an employee, working in section M2 of the facility performing lockout tagout on a plastic injection molding machine, verified the isolation and de-energization of the machine prior to starting work on the machine.

29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form.  a) Ventra Sandusky, LLC, 3020 Tiffin Ave. Sandusky. On or about July 13, 2015 the employer did not evaluate the respiratory hazard in the workplace in section M2 of the plant, when there was a potential for employees to be exposed to methyl methacrylate and ethyl acrylate which can cause respiratory illness, such as but not limited to, pulmonary edema and is considered a group 1B possibly carcinogenic in humans.
