29 CFR 1910.215(a)(4): Work rests. On offhand grinding machines, work rests shall be used to support the work. They shall be of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests shall be kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work rest shall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion.  Maintenance shop area- On or about March 17, 2014, the distance betwen the abrasive wheel and the adjustable work rest on the Dayton industrial grinder, manufacturer's serial number IZ707Y, exceeded one-eighth inch, thereby exposing employees to struck-by hazard(s) associated with rotating abrasive wheel and wheel breakage.

29 CFR 1910.215(b)(9): Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch. (See Figures O-18, O-19, O-20, O-21, O-22, and O-23.)  Maintenance shop area- On or about March 17, 2014, Dayton industrial pedestal grinder, manufacturer's serial number 1Z707Y,  did not have a tongue guard on the right side of grinding wheel, thereby exposing employees to struck-by hazard(s) from flying particles.

Maintenance shop area- On or about March 17, 2014, employees were required to use a metal knockout type outlet box that was not wall mounted. The outlet  box was being used as a 10 foot extension cord that was then plugged into a nearby wall mounted outlet. Thereby exposing employees to shock, electrocution, and fire hazard(s).

Hard-service cord and junior hard-service cord No. 14 and larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced.  Maintenance shop area and throughout facility- On or about March 17, 2014, employees were required to use Dyna-Glo pro portable electric heaters with 10 foot power cord(s) that had been spliced twice along their entire length. Exposed conductors could be visibly seen after black electrical tape was removed. Thereby exposing employees to shock and electrocution hazard.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) shall be visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated.  Steel blue tool cabinet number five (5)- On or about March 17, 2014, employees were required to use cord and plug equipment including, but not limited to power drill equipment, on a daily basis. The insulation on the electrical cord for the power drill was visibly damaged and was covered with black electrical tape. Thereby exposing employees to shock and electrocution hazard.
