29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective. On or about March 10, 2014, and at times prior thereto, the path to ground on the Peak Oilfield Services trailer no. 22999 that was at setup at Mabee Foundation "D" 6.5 miles NE from Midland servicing Sendero Rig No. 6 was not effective. This condition exposed employees to electrical hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws. On or about March 10, 2014, and at times prior thereto, a Peak Oil Services mobile home trailer no. 22999 was installed using an multi-conductor 120/240 VAC electrical power lines that was brought into the trailer's "J Box" without the use of a proper strain relief device that will prevent electrical wires from being pulled from terminal connectors. This condition exposed employees to electrical hazards.
