29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards:  a)    The Plant #6 air compressor mezzanine had gaps in the guard rails of approximately 24" & 15" exposing employees to a 14' fall hazard.  b)    The Washer filter platform had approximately a 32" wide unguarded opening approximately 11' off the cement floor.  c)   There was an approximately 11' high deck above the drying oven where employees go to change filters without fall protection.  d)   The Plant #7 truck loading dock has a drop off of approximately 4' outside of Dock #12 man door.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)  The #6 Assembly plant scissors lifts were not all guarded with bellows.  b)  There was approximately 23" to the nip points on the conveyor for the Pour & Debridge.  c)  Transfer table conveyors need guarding for nip point on the 10", 8", & 7" presses.  d)  The Tail stretcher for the 10" press had unguarded nip points.  e)  The Hartford 1440 lathe was missing the chuck guard in the machine shop.
