29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a) On or about March 2, 2025, at the establishment located at 2608 Ross Avenue, Schofield, Wisconsin; the employer did not ensure that employees entering the Main Line Cure Oven to clean up fallen louvers were trained as authorized employees under the control of hazardous energy, exposing employees to hazards, such as but not limited to mechanical and natural gas. b) On or about February 12, 2025, at the establishment located at 2608 Ross Avenue, Schofield, Wisconsin; the employer did not ensure that an employee entering the Main Line Blow Off Dry Oven to pick up fallen parts was trained as an authorized employee under the control of hazardous energy, exposing an employee to hazards, such as but not limited to mechanical, natural gas, and thermal. c) On or about February 11, 2025, at the establishment located at 2608 Ross Avenue, Schofield, Wisconsin; the employer did not ensure that employees entering the Main Line Cure Oven to rehang parts were trained as authorized employees under the control of hazardous energy, exposing employees to hazards, such as but not limited to mechanical, natural gas, and thermal.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6). a) On or about March 2, 2025, at the establishment located at 2608 Ross Avenue, Schofield, Wisconsin; employees were exposed to hazards, such as but not limited to mechanical and natural gas when entering the Main Line Cure Oven to clean up fallen louvers. The employer did not ensure that employees had implemented energy control application steps or equivalent protective measures prior to allowing them to enter the Main Line Cure Oven. The machine was not isolated, such as but not limited to electrical and natural gas, prior to the employees engaging in the servicing and/or maintenance activity. b) On or about February 28, 2025, at the establishment located at 2608 Ross Avenue, Schofield, Wisconsin; an employee was exposed to hazards, such as but not limited to natural gas when entering the Main Line Cure Oven to perform preventative maintenance work. The employer did not ensure that the employee had implemented energy control application steps or equivalent protective measures prior to allowing them to enter the Main Line Cure Oven. The machine was not isolated, such as but not limited to natural gas, prior to the employee engaging in the servicing and/or maintenance activity. c) On or about February 12, 2025, at the establishment located at 2608 Ross Avenue, Schofield, Wisconsin; an employee was exposed to hazards, such as but not limited to mechanical, natural gas, and thermal when entering the Main Line Blow Off Dry Oven to pick up fallen parts. The employer did not ensure that the employee had implemented energy control application steps or equivalent protective measures prior to allowing them to enter the Main Line Blow Off Dry Oven. The machine was not isolated, such as but not limited to electrical, natural gas, and thermal prior to the employee engaging in the servicing and/or maintenance activity. d) On or about February 11, 2025, at the establishment located at 2608 Ross Avenue, Schofield, Wisconsin; employees were exposed to hazards, such as but not limited to mechanical and natural gas when entering the Main Line Cure Oven to rehang parts. The employer did not ensure that employees had implemented energy control application steps or equivalent protective measures prior to allowing them to enter the Main Line Cure Oven. The machine was not isolated, such as but not limited to electrical and natural gas, prior to the employees engaging in the servicing and/or maintenance activity. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.
