29 CFR 1910.178(m)(1): Truck operations. Trucks shall not be driven up to anyone standing in front of a bench or other fixed object.  On or about March 19, 2025, at an establishment located at 880 South View Drive in Mosinee, Wisconsin, in the Packaging Department on the Semi-Auto Line; Packaging Support employees unloading aluminum parts off of carts onto a fixed roller table were exposed to struck and crushed-by hazards created by the Combi-Lift operators that were driving up to line to remove and replace the carts. The employees were standing in front of the fixed table when the Combi-Lift operators placed and removed the carts approximately 3 feet away.

Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.  On or about March 19, 2025, at an establishment located at 880 South View Drive in Mosinee, Wisconsin, in the Packaging & Shipping Departments; a CombiLift LIFT-062 (Model: O6000CB, Serial: 27976) with fork extensions did not have an updated operation plate to adjust the lifting capacity, exposing employees to tip over hazards.  Crystal Finishing Systems, Inc. was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1910.178(a)(4), which was contained in OSHA Inspection Number 1700628, Citation Number 1, Item Number 3-a, and was affirmed as a Final Order on March 25, 2024, with respect to a workplace located at 4704 Bayberry Street in Schofield, Wisconsin.
