29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. WORKSHOP: The employer had not established step-by-step hazardous energy control lockout/tagout procedures in the machine specific for the employees during servicing and maintenance of equipment, such as (but not limited to) the Line 1 and Line 2 pack down machines. On/or about January 5, 2014, the power source of the Line 2 pack down machine was not turned off and locked out prior to allowing an employee to perform cleaning on the rotary airlock of the fluidized bed during a product change over, and the employee sustained a tip of a finger amputation.
